Three things people try for a stiff neck, and why each one stops working

Every one of them works. Every one of them stops. The reason has nothing to do with which one you bought.

Almost every at-home neck device on the market falls into one of three categories. Once you can see the categories, the thing that has probably been confusing you — why each one worked, and then didn't — stops being confusing.

The three categories

Percussion and kneading devices. Massage guns, and the shoulder units with the rotating nodes. These apply mechanical force to soft tissue.

Passive positioning devices. Foam wedges and neck stretchers. These hold the neck in a position of extension. No power, no heat, about thirty dollars.

Thermal devices. Heating pads, wraps, the rice bag you microwave. These raise tissue temperature.

Each of the three does its one thing perfectly well. That is not the problem.

Why the muscle is tight in the first place

The muscle most often involved is the levator scapulae. It runs from the upper inner corner of the shoulder blade to the side of the neck, and its documented perpetuating factors read like a description of office work: forward head position, elevated shoulders, sustained static posture at a workstation, stress.

Trigger points in that muscle and in the upper trapezius refer pain in patterns people describe remarkably consistently — the base of the skull, the temple, behind one eye. Which is part of why a lot of people who arrive describing headaches leave talking about their neck.

Here is the part that matters. That muscle is not tight because it is weak, and it is not tight because it is knotted. It is tight because it is guarding — holding a joint that is under load, the way you would brace a shelf that is carrying too much. It is not a malfunction. It is the muscle doing its job.

You cannot massage a muscle out of doing its job. You have to take the job away.

How much load, exactly

A 2014 paper in Surgical Technology International modelled the forces on the cervical spine at different angles of forward head tilt. Roughly 12 lb with the head stacked over the shoulders. About 27 lb at 15 degrees. About 40 lb at 30 degrees.

Fifteen degrees is what reading this sentence looks like.

Two things need saying about that study, because they are usually left out. It is a computational model, not a measurement taken in living people, and it has been criticised on exactly that basis. We cite it because it is the clearest published estimate we know of, not as proof of anything about your neck.

So why does the massage stop working?

Because it negotiates with the guard while the load is still on the shelf. The muscle lets go — genuinely, and it feels good. Twenty minutes later it comes back, because nothing about the shelf changed.

Then most people conclude they bought the wrong device and go buy a slightly better one from the same category. That is the loop, and it is an expensive one.

The order a physical therapist works in

They do not start with the muscle. They start with heat.

Connective tissue does not lengthen when it is cold. The clinical literature puts the threshold at roughly a 4 °C rise in tissue temperature before collagen becomes meaningfully more extensible — which is why "warm up before you stretch" is a protocol rather than gym advice.

Then they take the load off. Not stretch it — unload it. A stretch pulls on a muscle that is still doing its job. Unloading gives it permission to stop.

Then, and only then, the soft tissue work — because by that point there is nothing left to guard.
